If you suffer from oversized breasts that create obstacles in your daily life, cosmetic breast reduction may be what you need to attain the silhouette you wish for. This surgery is ideal for women who have anxiety or are not able to live a fulfilled and active life because of the size of their breasts. If you also have ongoing skin inflammations or irritations (as well as neck pain), breast reduction surgery can reduce these annoyances. It is important patients are at a healthy weight, and choose a skillful surgeon like Dr. McNally who understands their concerns and goals and has the training to produce a pleasing and natural appearance.

At The Oregon Clinic and Réviance, we are able to perform breast reduction surgery as an outpatient service using general anesthesia. The time required for your procedure will depend on a variety of factors, including the type of incision needed, the general technique involved, and the amount of breast tissue that needs to be removed. The procedure starts with an incision, which will be made around the areola and down to the fold underneath the breast (in a vertical pattern) or as a full anchor shape around the areola and down and around the folds under the breasts (called the Wise pattern). Dr. McNally will review these choices during your consultation, but the technique used will depend on your preoperative breast size, the position of your nipples, and your cosmetic goals. The areola and nipple will also be repositioned as necessary for a natural appearance. While scarring is common from this procedure, advances in surgical methods have led to a dramatic decrease in visible scarring from breast reduction surgery.

After surgery, our patients will be observed closely in our private recovery room and then sent home to begin the healing process. A thin tube for draining is usually inserted near the surgical site to assist with swelling and bleeding. A compression bra will be provided for several days to improve blood circulation to the breasts during the initial healing period. Within about a month, the breasts will settle, and the post-surgery effects will go away. Most of our patients at The Oregon Clinic and Réviance report they feel lighter, relieved, and more confident. Following surgical breast reduction, you can enjoy the clothes you have always wanted and feel confident with your new appearance. Remember, it is still important to stay at a healthy body weight so the outcomes are not compromised.

I have had large breasts since I can remember and when it finally came around to me making an appointment for a breast reduction, I was definitely nervous. I’m only 21, but my doctor had recommended doing it sooner rather than later. Dr. McNally was super helpful and answered all my questions. I felt very comfortable and as I began this process during COVID, I felt very safe. He made sure to set my expectations for everything and that helped the process go very smoothly, especially because it can be hard to get breast reduction covered by insurance (although I was lucky and got approved first round). I am now one week post-op and feeling great. He was quick and while my recovery and healing process will take some time until they look “normal,” they’re looking great so far and I’m so happy with my choice and my results. Would highly recommend!

I got a breast reduction with Dr McNally after a friend recommended me to him for gender affirming care. From beginning to end, I felt so safe, affirmed and comfortable. McNally and his entire team are warm & very comforting people in an environment that is typically very anxiety inducing. I’ve been beyond pleased with my results and the level of care I’ve received throughout the last 9 months. I’m sending anyone and everyone I know who needs surgery his way! Thanks Sean!

I have wanted/needed a breast reduction for most of my life. I had insecurities about my body but more importantly I experienced chronic back pain. I tried everything before reaching out to Dr. McNally for his opinion. Sean McNally has such a kind yet comical nature; he made me feel heard, reassured and so at ease through everything.. You can tell this comes as a second nature to him. I am still in disbelief with how easy the process was leading up to my surgery and how amazing his team has been throughout my recovery. My new breasts look absolutely beautiful and have already made me feel more comfortable and confident than ever before.

I can’t say enough good things about Dr. McNally and his PA, Whitney. I first worked with Dr. McNally during a breast reduction that was part of my breast cancer surgery, and later again for my mastectomy, where he began my reconstruction and placed expanders. I’ll be continuing with him for my implant exchange, and I truly wouldn’t trust anyone else with my care. From the very beginning, Dr. McNally stood out not only for his knowledge and surgical skill, but for the way he approaches patient care as a whole. He took the time to carefully consider every decision through the lens of my cancer diagnosis, always making sure that my health and long-term outcome were the top priority. That level of thoughtfulness and intention made a difficult process feel much more manageable. He’s also incredibly personable—his sense of humor helped ease a lot of anxiety during an otherwise overwhelming time.Whitney is equally exceptional. I’ve had multiple appointments with her, and she has a natural ability to make you feel comfortable and at ease almost immediately. She is kind, attentive, and extremely knowledgeable, and she takes the time to answer questions in a way that makes you feel informed and supported. You can tell she genuinely cares about her patients and takes pride in the care she provides.Together, Dr. McNally and Whitney make an outstanding team. Their communication, compassion, and expertise create an environment where you feel truly cared for and confident in your treatment plan. I’m incredibly grateful to have them as part of my care team and would highly recommend them to anyone navigating a similar journey.

About Us
Will my insurance cover breast reduction?

In some cases, your health insurance might cover part of the cost of breast reduction surgery if it's considered medically necessary. If you are considering using your insurance, talk to Dr. McNally so he can help you with your claim. You should talk to your provider to find out if your specific policy may pay for the surgery and the information they need to approve it. Your insurance will most likely decide based on measurements like your height, weight, body surface area, and body mass index (BMI).

Will my nipples be moved?

Usually, the nipples and areola will be moved up to a central position on the breast during breast reduction surgery. If a significant amount of breast tissue and skin needs to be removed, your nipple-areola are may be cut out and moved. Dr. McNally will choose an incision technique so the nipple and areola are easy to move higher. It is also possible to reduce the size of the areola with an areolatome. All of these options will be discussed during your consultation before Dr. McNally helps you choose the best plan to meet your goals.

Can I breastfeed after a breast reduction?

Reduction surgery can affect your ability to breastfeed. While some women may still be able to breastfeed after a surgical reduction, some cannot. It is difficult to predict whether your surgery will interfere with breastfeeding. This is why Dr. McNally suggests that patients be done having children or be willing to not breastfeed before considering breast reduction surgery.

Will I have scarring after a breast reduction?

Scarring is an inevitable part of surgery, but it should not be very visible thanks to modern surgical techniques. The most common incision patterns are a vertical line down to the breast fold or an anchor shape around the areola and underneath the breast. Dr. McNally will discuss the best incision plan to meet your goals while minimizing scarring.

Will my breasts look natural after breast reduction?

Yes, the goal is to create a smaller, perkier, and natural-looking breast shape. At The Oregon Clinic and Réviance, we will reposition the nipple and areola as needed to ensure the final result is proportionate to your body. Our extensive training is focused on producing aesthetically pleasing and natural outcomes.

Will I lose sensation in my nipples after breast reduction?

While changes in nipple sensation can occur, they are often temporary. The surgical technique used plays a role, and our team takes every precaution to preserve nerve function. Many patients in Portland, OR, find that sensation returns to normal within a few months to a year as the nerves heal.
